Delayed postanoxic leukoencephalopathy (DAL) is reported with intoxication by carbon monoxide, narcotics, along with other toxins. OBJECTIVE–To investigate the metabolic derangement of the white matter (WM) and bloodbrain barrier (BBB) immediately after DAL caused by methadone overdose. Design, SETTING, AND PATIENTS–Case report of 2 patients with DAL immediately after just one dose of “diverted” methadone used for discomfort control. RESULTS–In each circumstances brain magnetic resonance imaging (MRI) unveiled preliminary substantial bilateral limited diffusion lesions inside the WM. Follow-up MRI applying proton magnetic resonance spectroscopic imaging (1H-MRSI) showed markedly lower N-acetylaspartate and larger choline within the WM.
